How to Claim for Brazilian blowout class action settlement?

Have you ever visited a salon to get a Brazilian Blowout hair treatment? Or have you purchased any of the Brazilian Blowout products like solution or the Acai professional smoothing solution? If yes, then you may be entitled to claim some money under the class action settlement. Back in year 2010, a class action lawsuit was filed by California Attorney General Jerry Brown and the people of California against the popular hair products company - Brazilian Blowout. The reason behind the lawsuit is the formaldehyde-content present in the products. According to plaintiffs, Brazilian Blowout allegedly told that their products are formaldehyde free and they contain no harsh chemicals, but in reality these products contain formaldehyde and other toxic ingredients.

Later on 5th June 2013, a federal judge granted preliminary approval to a settlement valued at $4,225,000 between Brazilian Blowout Company and plaintiffs. So, you could receive the benefits of this Class Action Settlement only if: (i) you are a resident of the United States who bought Brazilian Blowout Products either directly from GIB, LLC or from one of its approved distributors on or before 6th June, 2012, or (ii) you are a resident of the United States who underwent a hair treatment with Brazilian Blowout Products on or before 6th June, 2012.

According to the Brazilian Blowout settlement website, stylists can claim and get $75 for each bottle of the Brazilian Blowout product they bought. And if they want they can even forfeit their cash refund and exchange the unopened bottles with a bottle of new Brazilian Blowout Zero. Moreover, consumers can claim for the Settlement payment of $35 for Brazilian Blowout hair treatment they received. They can claim for up to a maximum of three treatments (i.e. $105).

Stylists and consumers are also eligible to claim a payment of up to $2,000 in medical reimbursement, if they have legal records that demonstrate medical expenses incurred due to some short-term symptoms of formaldehyde exposure. And your treatment should not last for more than ten days. The qualifying symptoms/conditions for medical reimbursement include:
