Coventry is a health maintenance organization that maintains a network of providers including hospitals, doctors, etc. A settlement has been reached with company, Coventry Health Care. The class action lawsuit is about the compensation for certain workers' compensation medical bills. The lawsuit claims that the company incorrectly determines the under workers’ compensation rules whenever a supplier submits a billed charge for a service that is below the specified amount for that service under the relevant state’s fee schedule. Those who are eligible for this settlement can file a claim and get benefits.

Who is Eligible?

You are eligible for settlement, if you are health care providers who:

1. Have/had a First Health provider agreement having an Appx A that uses the phrase “maximum amount payable” in the language about reimbursement for services offered to occupationally ill or injured workers; and
2. Have had bills discounted under that provider agreement from March 25, 1999 to Sept. 3, 2014 by Coventry’s discounting the billed charge when that charge was less than the state or federal fee schedule amount.

The settlement is divided into three groups. They are Notified Class Members, Self-Identified Class Members, and Actual Damages Class Members.

Coventry denies all allegations but has agreed to settle the class action lawsuit to avoid the cost and risk of a trial.

How to File a Claim?

First of all, if you received a notice about the Coventry class action settlement in the mail, you do not need to do anything to get benefits. However, you must submit a Claim Form if you wish to claim additional settlement benefits.

If you haven’t received notice you can file a claim by following way:

To participate in the settlement you must visit at www.coventrysettlement.com. On the left site of the webpage, click on the tab “CLAIM FORM”. Follow the links and instructions to complete and submit the claim form with the supporting documentation including copies of sections of your First Health PPO provider agreement and applicable appendix.

The benefits that all eligible individuals could receive are:

• If you receive a notice about this settlement in the mail, you will automatically receive a payment of $50 without submitting a Claim Form.
• Class Members who submit documentation will receive a proportional share of the $5 million Guaranteed Fund after all Notified Class Members are paid.
• Actual Damages Class Members who submit documentation will receive up to the amount of their actual damages from the Individual Claim Fund. Coventry has decided to give up to $3.5 million to the Individual Claim Fund.
