At first, select from the 3 categories of individual claims for physical injury or death:
1. Claims for Death
2. Category One: Physical Injury Claims:
Physical injury claims relating quadriplegic injury, paraplegic injury, double amputation, permanent brain damage needed continuous home medical assistance, and pervasive burns encompassing a substantial part of the body ("Category One Claims").
3. Category Two: Physical injury claims other than Category One Claims:

The Claim Form submission period starts on August 1, 2014, and make sure that all claims must be postmarked or submitted by December 31, 2014. Once you file a claim, you will be given a unique Claimant Identification Number right away. You can use that number to log into the system and generate your unique password that helps you track the status of your claim.

For being eligible to make a claim under the Facility, you must have been a driver, passenger, pedestrian or an occupant of another vehicle caught up in an accident resulting in physical injury or death because of the Ignition Switch Defect in one of the following categories of vehicles ("Eligible Vehicle"):

Service Part Vehicles

The list includes vehicles, in which the ignition switch was replaced with an ignition switch bearing Part No. 10392423 before the accident; and the accident in question happened after such replacement.

You also have to submit documentation e.g., an official police report, the Event Data Recorder ("EDR") data captured by a vehicle's computer, photographs, insurance claim and report of the accident, repair and warranty records, etc. to prove that the Ignition Switch Defect in an Eligible Vehicle was the proximate cause of the accident resulting death or physical injury. Complete list for the supporting documentation will be clearly mentioned on the Claim Form.

All Claim Forms with the provided documentation are reviewed by the Facility Administrator and his staff to decide the eligibility of the claim as well as the amount of compensation in accordance with the Protocol. The Facility will use the calculation methodologies to decide the amount of compensation to be paid.
To receive economic loss compensation, a claimant submitting death claim is asked to select between tracks A & B. Track A is for Presumptive Compensation and Track B is for Complete Economic Analysis. For non-economic loss in both tracks, Claimant submitting a death claim will receive

$1,000,000 for the death of the decedent
$ 300,000 for the surviving spouse
$ 300,000 for each surviving dependent of the decedent

Besides death claim, economic loss compensation for claimants submitting claim of a Category One Physical Injury will be calculated in the same way under Track A or Track B.
